Special Olympics New Jersey (SONJ) receives funding from a variety of sources, each with its own reporting requirements. To fulfill our obligations and ensure continued support, we request data from participating schools in several formats throughout the year. Your timely and accurate reporting is essential to our shared success. Please submit a separate report, survey, and tracker for each school and keep in mind that late, incomplete, and missing reports negatively affect your school’s standing with the UCS grant. 

Monthly Reports

  • Due Date: 10th of each month (for the previous month)
  • Location: The report will be available on this page and in the Monday email around the 1st of each month.
  • Requirement: All schools should submit a report each month, even if no Unified activities took place.
  • Purpose: These reports track the number of individual athletes and partners who participated, as well as the number of experiences per participant. Each experience is counted separately for each athlete and partner.
  • Mechanism: GoogleForm

Activity Tracker

  • Due Dates: January (for the first half of the school year) and June (cumulative for the full year)
  • Purpose: The Activity Tracker captures participation and experiences differently than the monthly reports. It provides a broader overview of Unified programming across the year.
  • Mechanism: GoogleForm distributed in the UCS weekly email and accessible on this page

UMass Survey

  • Due Date: May 12, 2026
  • Administered by: University of Massachusetts–Boston
  • Purpose: This national survey helps Special Olympics North America (SONA) evaluate the impact of Unified Champion Schools and secure federal funding for continued implementation.
  • Importance: The completion rate of this survey directly affects the level of funding SONJ is eligible to receive. If we do not maintain or exceed our current response rate, our funding may be reduced.
  • Mechanism: UMass will email each school’s primary liaison directly.
